Vaccination history for all boarding patients will be required at admission. These vaccines must be given by a licensed Veterinarian and not obtained from a feed store or other sources. If medical records are not presented at time of admission, a charge of $5.00 will applied to your account for the labor necessary to retrieve the needed medical records. If vaccines are overdue, the required vaccines will be given at owner’s expense. If refusal of vaccines is made, we will not admit the pet into our hospital.
Dogs: Current on Rabies, Distemper, Parvo Virus, and Bordetella
Cats: Current on Rabies, Feline Distemper, Rhinotracheitis, Calici Virus, and Feline Leukemia. All cats must be FIV/FELV negative. Status of this test must be presented in the vaccination history.
All pets must have a Fecal Exam within the last 12 months and be free of external parasites

PICKUP/DROP OFF AND EMERGANCY POLICY
Animals will be admitted & discharged ONLY during regular office hours. Monday-Friday 7:30am-10:00pm, Saturday 8:00am-2:00pm. If you require drop off or pick up after normal business hours a release fee of $50.00 will be added to your account at time of discharge.
Any animal that requires emergency or immediate veterinary attention will receive it at our discretion and at the owner's expense.
We feed Science Diet Sensitive Stomach for easy digestion. If your pet is on a special diet, please provide enough food for the duration of your pets stay. If your pet runs out of a special prescription diet during its stay, you have the option to refill your pet’s food at retail price.
Southwest Animal Hospital is not responsible for any damages to any belongings such as bags, bedding, blankets, carriers, leashes, collars, etc., during your pet’s stay.
I acknowledge that all payments are due at the end of every service. I agree to pay the entire balance for all services rendered at time of discharge.
