Grooming Policy

VACCINATION POLICY

All grooming patients entering Southwest Animal Hospital must be current on required vaccinations and blood work. We would be happy to contact your previous veterinarian to retrieve your pet’s medical records. In the event extensive efforts are made to locate medical history, a charge of $5.00 will be applied to your account. If you are unable to provide vaccination history, the patient will be considered overdue on vaccinations. If vaccines are overdue, the required vaccines will be given at the owner’s expense. If refusal of vaccines is made, we will not admit the pet into our hospital. Vaccines are only valid if given by a licensed Veterinarian.

REQUIRED VACCINES

DOGS: Rabies, Distemper Combo Vax, Parvo, and Bordetella vaccines. Parvo Test (at Dr.’s discretion)

CATS: FIVLV Test/Status, Rabies, Feline Upper Respiratory Combo Vax (RCPP), Feline Leukemia.

FECAL: Fecal Examination for internal parasites within the last 12 months

EXTERNAL PARASITES: If fleas & ticks are present, treatment will be performed upon admission at the owner’s expense.

GROOMING HOURS

Grooming appointments are available Monday thru Friday. Drop off times are scheduled between 7:30am-8:00am. Discharge times are scheduled after 4:00pm.

If you need to drop off or pick-up after normal business hours, a fee of $50.00 will be added to your account at time of discharge.

SATISFACTION

If you are unhappy with a groom at anytime, please notify us within 24 hours so the problem can be fixed free of charge.

PAYMENT

I acknowledge that all payments are due at the end of every service. I agree to pay the entire balance for all services rendered at time of discharge.
